Fill out and submit application below.

All students must submit a $45 non-refundable processing fee with their application (after submitting the form below, you will be directed to PayPal).

Submit audio or video recordings of two contrasting pieces, meeting the minimum level requirements.

New students must arrange to have a letter sent from their private or school music teacher. Letters should be emailed to: director@musicatportmilford.org, or mailed to:
